Supplemental Waiver Application
This application is for families who wish to apply for a Supplemental Income Waiver.
The information that families provide on this application and any supporting documentation will be kept by Braintree Public Schools. It will only be used by Braintree Public Schools or the Massachusetts Department of Elementary and Secondary Education to verify household income to determine whether families would be eligible for a waiver of reduction of certain school fees.
Examples of programs which may be eligible for a waiver or reduced fee:
Bus pass for students who are eligible for transportation
Braintree High School Athletics
Full Day Kindergarten tuition fees
Integrated Preschool
In order to qualify, please review the eligibility criteria and complete the application. As outlined, you are required to provide all identified income documents.
Application and supporting income verification documents should be returned to: Karen Flaherty, karen.flaherty@braintreeschools.org or dropped in person at the Administration building located at: 348 Pond Street.
Due to confidentiality, please do not submit the application or any supporting income documents to the home schools.

Families will be notified of eligibility determination within 1-2 weeks after receiving application and all supporting income documentation.
