Student Teaching / Observation

A District Reminder

Relative to Student Teachers, Student Observers and Internship Requests

  • The college or university must submit a letter on their letterhead requesting the placement and include a recent copy of the student’s transcripts.The request for placement and transcripts should be directed to the Assistant Superintendent of Schools and can be faxed to 781-380-0146.The request and transcripts will be forwarded to the appropriate director and/or principal.

  • The Assistant Superintendent’s office will then confer with the appropriate principal and/or director to determine if the request can be accommodated.The principal and/or director should schedule an interview with the potential student teacher/observer, and discuss the possible placement with the supervising teacher.

  • The student is then instructed to make an appointment to complete the required forms at the office of the Assistant Superintendent. These forms include a CORI request (copy of license required), a Student Teaching/Observation form, and an emergency card. The Student Teaching/Observation form will be forwarded to the appropriate principal and/or director for approval and should be returned to the office of the Assistant Superintendent.

  • Once all forms have been submitted, the CORI has been processed and fingerprinting results have been received, the student teacher and the principal and/or director would be contacted by the office of the Assistant Superintendent indicating that the placement has been approved and the scheduled can be arranged between the supervising teacher and the student.

Become A Substitute

Before an interview is scheduled, prospective substitutes must submit the following:

After being interviewed, the following must be received from all subs:

  • CORI authorization (be sure to note the last six digits of your social security number and sign the back; a government issued photo id is needed to process this form)
  • Copy of your fingerprinting receipt or a suitability letter from another MA school (Sign up to have your fingerprints taken if you have not previously done so; when registering please be sure to select "PreK-12 Grade Education" as the "agency/sector")
  • Ethics acknowledgment & certificate of completion (online training link: http://www.muniprog.eth.state.ma.us/ )(please note: this site does not work in Google Chrome, on Chromebooks, on smartphones or on tablets; be sure to disable pop-up blocker prior to starting training)
  • Physician’s certificate (If you have had a physical within the past year, you do not have to have another physical; however, your physician would need to complete this form. If your doctor deems you low risk for TB, you do not need to have the TB test.)
  • I9 (as outlined on the I9, two documents of identification must accompany this form)
  • Payroll information
